Overview
- The Clinical Frailty Scale (CFS) is a judgement-based 9-point scale summarising overall fitness or frailty after clinical assessment. Higher scores indicate greater frailty and risk.
- For acute illness, score the patientβs pre-morbid baseline (not current acute state), except grade 9 (terminally ill) where current state applies.
- Commonly used in geriatrics, emergency medicine, oncology, perioperative assessment, and frailty screening. Tap the grade that best matches the description in the tool above.
Scoring tips
- Requires clinical judgement β match the written description, not pictorial examples alone.
- If a patient fits two levels equally, score the higher (more dependent) level.
- Grade 9 applies when life expectancy is <6 months from a defined illness, regardless of prior baseline frailty.
