When to replace
- Phosphate is critical for cellular energy and membrane function.
- Replace when clinically significant deficiency, especially refeeding, DKA recovery, or symptomatic hypophosphataemia.
Cautions
- Do not treat hyperphosphataemia with replacement — address cause.
- Monitor calcium and magnesium during correction.
- IV phosphate requires local protocol and renal caution.
